Job Description

Job Title: Home Health Nurse

Job Location: Texas

Type of Facility: Critical Access Hospital with Home Health Services

Length of Assignment: 6 weeks 

Start Date: Immediately

Work Schedule: Monday-Friday, 8:00 AM - 5:00 PM

Roles and Responsibilities:

- Conduct home visits to patients

- Perform start of care assessments, recertifications, and routine visits

- Provide care to a geriatric patient population

- Manage a patient ratio of 3-10 patients per day

- Utilize OASIS documentation and the facility's EMR system

- Work independently in a home health setting

- Collaborate with the healthcare team to ensure quality patient care

- Adhere to all safety protocols and healthcare regulations

- Utilize company-provided vehicles for traveling between patient homes

Requirements:

- Current Texas State RN License

- BLS Certification

- Minimum of 1 year of home health experience

- OASIS experience
