Job Description

Job Title: Home Health Nurse

Job Location: Washington

Type of Facility: Skilled Nursing Facility, Hospice and Rehabilitation Center

Length of Assignment: 13 weeks

Start Date: Immediately

Work Schedule: Five 8-hour shifts per week, 8:00 AM - 4:30 PM

Roles and Responsibilities:

- Assess, diagnose, plan, implement, and evaluate care delivery for home health/hospice rehab patients

- Provide direct patient care and supervise care provided by professional and ancillary personnel

- Maintain a patient ratio of 1:4

- Assess and plan for patient and family educational needs to support health maintenance and restoration

- Collaborate with the interdisciplinary team on care plans

- Utilize Cerner EMR system for documentation

- Float to other departments as needed

- Adapt to a diverse patient population across all age groups

- Maintain accurate and timely patient records

- Communicate effectively with patients, families, and healthcare team members

Requirements:

- Current Washington state RN license

- Minimum 2 years of experience in home health/skilled nursing

- ACLS, TNCC, PALS, and NIHSS certifications

- Emergency Service Index training
