Job Description

Job Title: Hospice/Palliative Care Nurse

Job Location: California

Type of Facility: Home Health and Hospice

Length of Assignment: 13 weeks

Start Date: Immediately

Work Schedule: 8:30 AM - 5:00 PM shifts

Roles and Responsibilities:

- Support Nurse Case Manager in patient care

- Communicate with Team Leaders, HHAs, and other providers as needed

- Instruct caregivers, patients, volunteers, and employed caregivers based on the Plan of Care

- Perform nursing procedures as requested

- Maintain documentation as per policies and regulations

- Provide care for 4-5 patients per day, primarily 55+ years old

- Travel up to 40-60 miles a day to patient homes and nursing facilities (mileage reimbursed)

- Utilize Oasis or Epic EMR system (experience is a plus)

Requirements:

- Active California RN license

- Bachelor's or Associate's degree in nursing

- BLS certification through Red Cross or American Heart Association

- Minimum 1 year of recent hospice experience (more experience preferred)

- Proficiency in home health and hospice care

- Excellent communication and patient care skills

- Ability to work independently in various settings
