Job Description

Job Title: Nurse Case Manager

Job Location: Alaska

Type of Facility: Indian Health Service (IHS) Facility

Length of Assignment: 13 weeks

Start Date: Immediately

Work Schedule: Monday-Friday 8:00 AM - 5:00 PM, with rotating Saturdays

Roles and Responsibilities:

- Assess, plan, implement, and evaluate processes for targeting health status of assigned patient panels

- Utilize advanced knowledge and skills in health process management and individual patient case management

- Provide oversight and development of processes related to coordination of health care services

- Work collaboratively with providers and CMAs in a team-based care model

- Manage a patient load of 7-13 patients per day

- Utilize Cerner EMR system

- Ensure quality care to meet comprehensive care needs of the patient population

- Participate in rotating Saturday shifts as scheduled

Requirements:

- At least 3 years of clinical experience (IHS experience preferred)

- Current Alaska RN license

- BLS certification
