Job Description

Job Title: Nurse Case Manager

Job Location: Ohio

Type of Facility: Home Health Hospice

Length of Assignment: 13 weeks

Start Date: Immediately

Work Schedule: Monday-Friday, 8:30 AM - 4:30 PM

Roles and Responsibilities:

- Develop and evaluate holistic care plans based on individual patient needs

- Customize care plans for each patient and maintain clinical expertise

- Apply psychiatric nursing skills as needed

- Promote patient participation in health management

- Coordinate with community agencies for care and discharge planning

- Use nursing process for data collection, planning, and care evaluation

- Manage a caseload of 5-7 patients per day

- Travel 25-40 miles daily (IRS mileage reimbursement tracked in EMR)

- Seek learning opportunities to stay current with nursing skills

Requirements:

- Ohio RN License

- BLS and CPR Certifications

- 2+ years of mental health nursing experience

- 1 year of medical-surgical experience preferred

- Home health experience preferred

- Spanish language skills are a plus
