Job Description

Job Title: Pharmacist

Job Location: Arizona

Type of Facility: Outpatient Pharmacy

Length of Assignment: 13 weeks

Start Date: Immediately

Work Schedule: Monday-Friday, 9am-6pm or 8am-5pm

Roles and Responsibilities:

- Review and verify prescriptions using ScriptPro machine and electronic counter

- Counsel patients on medications, their administration, and potential side effects

- Process and fill approximately 2800 prescriptions per month

- Review medication orders for appropriateness, safety, and completeness

- Consult with healthcare providers regarding discrepancies in medication orders

- Provide drug information to healthcare professionals and patients

- Participate in determining disease state management with physicians

- Review and advise on non-formulary and restricted medications

- Compound medications as prescribed and determine appropriate formulations

- Conduct patient education on medications and disease state management

- Inspect processed medications for accuracy and integrity before dispensing

- Report and document medication errors and interventions

- Perform monthly inspections of drug storage areas

- Collaborate with 1-2 other pharmacists and a pharmacy technician

Requirements:

- Bachelor's degree in Pharmacy or Pharm.D. from an accredited institution

- Active Arizona Pharmacist license (assumed based on location)

- Minimum 2 years of experience as a pharmacist

- BLS certification
