Job Description

Job Title: Pharmacist

Job Location: Arizona

Type of Facility: Outpatient Pharmacy

Length of Assignment: 13 weeks

Start Date: Early April

Work Schedule: Monday-Friday, 9am-6pm or 8am-5pm

Roles and Responsibilities:

- Review, process, and fill prescriptions (approximately 2800 prescriptions/month)

- Counsel patients on medications, administration, and potential reactions

- Review medication orders for appropriateness, safety, and completeness

- Consult with healthcare providers regarding discrepancies in medication orders

- Provide drug information to healthcare professionals and patients

- Participate in determining disease state management with physicians

- Review and advise on non-formulary and restricted medications

- Compound medications as prescribed

- Conduct patient education on medications and disease state management

- Inspect processed medications for accuracy and integrity

- Report and document medication errors and interventions

- Perform monthly inspections of drug storage areas

- Operate ScriptPro machine and electronic counter

Requirements:

- Bachelor's degree in Pharmacy or Pharm.D. from an accredited college or university

- 2+ years of experience as a Pharmacist

- Valid Arizona state Pharmacist license

- BLS certification
