Job Description
Job Title: Clinical Pharmacist
Job Location: Texas
Type of Facility: Hospital
Length of Assignment: 13 Weeks
Start Date: Immediately
Work Schedule: Night Shift, 8:30pm - 8am (exact days to be determined)
Roles and Responsibilities:
- Verify medication orders and IV products
- Provide medication dosing consults for vancomycin, aminoglycosides, warfarin, enoxaparin, and acetylcysteine
- Participate in antimicrobial and opioid stewardship programs
- Implement automatic protocols such as IV to PO conversions and renal dosing adjustments
- Collaborate with night shift pharmacy team (1 other pharmacist and 2 technicians)
- Ensure compliance with all pharmacy regulations and hospital policies
Requirements:
- Active Texas Pharmacist License
- IV Certification
- Minimum 1 year of hospital pharmacy experience
- Proficiency in medication order verification, IV product verification, and dosing adjustments
- Experience with vancomycin, aminoglycoside, warfarin, and heparin dosing
- Ability to work independently during night shifts
