Physical Therapist – Arizona

Job Description

Job Title: Physical Therapist (PT)

Job Location: Arizona

Type of Facility: Skilled Nursing Facility 

Length of Assignment: 13 weeks

Start Date: Mid May

Work Schedule: Monday - Friday, 8 AM - 5 PM

Roles and Responsibilities:

- Provide physical therapy services to post-acute patients

- Manage a caseload of 8-10 patients per day (occasionally up to 11-12)

- Conduct individual and group therapy sessions

- Maintain a minimum productivity of 80%

- Treat patients in 30-45 minute time slots

- Provide care for patients staying 7-28 days depending on their post-surgical needs

- Work with both inpatient and outpatient populations

- Accurately document patient care using NetHealth EMR system

- Collaborate with interdisciplinary team members

Requirements:

- Active Arizona PT license or Compact State license

- BLS certification

- Minimum of 1 year of experience as a PT

- Strong clinical skills in post-acute care
