Job Description

Job Title: Physical Therapist (PT)

Job Location: Arizona

Type of Facility: Skilled Nursing Facility

Length of Assignment: 13 weeks

Start Date: Immediately

Work Schedule: Monday-Friday, 8:00 AM - 5:00 PM

Roles and Responsibilities:

- Provide physical therapy services to 8-10 patients daily (occasionally up to 11-12)

- Maintain a minimum productivity of 80%

- Conduct therapy sessions in 30-45 minute time slots

- Participate in group therapy sessions when required

- Treat post-acute patients recovering from various surgical procedures

- Work with both inpatient and outpatient cases, depending on patient needs

- Utilize NetHealth EMR system for documentation

- Adapt to patient stays ranging from 7-28 days based on surgical procedures

- Possibility of occasional Saturday overtime to manage weekly overload

Requirements:

- Active Arizona PT license (or compact state license, 12-16 weeks processing time)

- BLS certification

- Minimum 1 year of experience preferred
