Job Description
Job Title: Home Health Physical Therapist
Job Location: California
Type of Facility: Home Health Agency
Length of Assignment: 13 weeks (with potential for extension)
Start Date: Immediately
Work Schedule: Monday-Friday, 9am-5pm or 4 days a week
Roles and Responsibilities:
- Conduct initial evaluations, starter care, and discharge visits for home health patients
- Manage a caseload of 6-7 patients per day
- Provide physical therapy services to patients aged 60 and up, primarily Medicare beneficiaries
- Collaborate with PTAs who manage ongoing treatment plans
- Maintain accurate and detailed patient records
- Complete documentation using personal smartphone or laptop
Requirements:
- Active California PT license
- At least 1 year of PT experience
- BLS + CPR certification
- Valid driver's license, car insurance, and vehicle registration
- Comfortable using technology for documentation and communication
