Job Description
Job Title: Physical Therapist
Job Location: California
Type of Facility: Home Health
Length of Assignment: 13 weeks
Start Date: Late January
Work Schedule: Monday-Friday, 8:30 AM - 5:00 PM
Roles and Responsibilities:
- Drive up to 60 miles to patients' residences and evaluate their care needs
- Help patients regain normalcy and improve their quality of living
- Communicate effectively with the home health care team
- Maintain accurate assessment and progress documentation
- Utilize company-provided equipment, including a phone and possibly a tablet
- Manage a caseload of 4-5 patients per day, primarily aged 50 and above
Requirements:
- Valid California PT license
- COVID-19 vaccine and booster, as well as flu vaccine
- Extensive experience in physical therapy (more years preferred)
- Previous travel experience highly desired
- Home health/home care experience required
- Epic EMR system experience strongly preferred
- Willingness to travel between 2 counties
