Job Description
Job Title: Physical Therapist (PT)
Job Location: California
Type of Facility: Skilled Nursing Facility
Length of Assignment: 13 weeks
Start Date: Early March
Work Schedule: Monday-Friday, 8:00 AM - 5:00 PM
Roles and Responsibilities:
- Provide licensed physical therapy services to 6-7 patients daily
- Conduct patient evaluations as ordered
- Develop and implement treatment plans aligned with patient benefits and outcomes
- Complete accurate and timely documentation using Point Click Care (PCC) and Optima systems
- Ensure patient safety using professional judgment
- Communicate patient status to staff and foster interdisciplinary communication
- Make appropriate referrals within the rehab team
- Utilize available equipment including ACP, modalities, and East Steam UT as needed
Requirements:
- Valid California PT license
- CPR certification
- Working knowledge of Medicare and similar funding sources
- Familiarity with Point Click Care (PCC) and Optima EMR systems
