Job Description
Job Title: Physical Therapist (PT)
Job Location: California
Type of Facility: Skilled Nursing Facility
Length of Assignment: 13 weeks
Start Date: Immediately
Work Schedule: Monday-Friday, 8:00 AM - 5:00 PM
Roles and Responsibilities:
- Provide licensed physical therapy services to patients aged 55+
- Conduct evaluations as per orders
- Develop and implement treatment plans aligned with benefits and outcomes
- Treat 6-7 patients per day, typically for 40-45 minute sessions
- Work with Medicare Part A and/or B patients
- Utilize Point Click Care (PCC) for nursing and medicine documentation
- Communicate patient status to staff and foster interdisciplinary communication
- Make appropriate referrals within the rehab team
- Utilize available equipment including ACP, modalities, and East Steam UT as needed
Requirements:
- Valid California PT license
- CPR certification
- SNF experience
