Job Description

Job Title: Physical Therapist (PT)

Job Location: North Carolina

Type of Facility: Home Care Agency

Length of Assignment: 13 weeks

Start Date: Immediately

Work Schedule: Flexible, typically weekdays during working hours with potential for Saturday appointments

Roles and Responsibilities:

- Conduct home visits within a 50-mile radius

- Utilize EVV (Electronic Visit Verification) system as required by North Carolina

- Review caseload, files, and schedule visits directly with patients (approximately one week in advance)

- Develop and implement personalized plans of care based on patient needs

- Communicate with office staff regarding necessary orders and patient progress

- Provide care for a diverse adult patient population, including VA patients, post-operative cases, diabetes/ulcer management, homebound individuals, and wound care

- Maintain flexibility in scheduling to accommodate patient needs

- Collaborate with office manager and other staff members as needed

Requirements:

- Valid North Carolina PT license or compact license

- Minimum of 2 years of experience, preferably in home health setting

- Ability to work independently and manage a flexible schedule

- Excellent communication and documentation skills

- Willingness to travel within the specified service area
