Job Description
Job Title: Physical Therapist (PT)
Job Location: Tennessee
Type of Facility: Home Health Agency
Length of Assignment: 8-13 weeks
Start Date: Immediately
Work Schedule: Monday - Friday, 8:00 AM - 4:30 PM
Roles and Responsibilities:
- Conduct evaluations, assessments, and create treatment plans for patients in their homes
- Provide physical therapy services to primarily geriatric patients, with some adult cases
- Perform patient discharges and complete necessary documentation
- Utilize HomeCare HomeBase (Point Care app) for electronic medical records
- Supervise Physical Therapy Assistants (PTAs) as needed
- Complete OASIS assessments as required by CMS
- Manage a caseload of 3-6 patients per shift
- Collaborate with interdisciplinary team including LPNs, RNs, social workers, SLPs, OTs, and behavioral nurses
- Adhere to all home health protocols and safety standards
- Provide home exercise programs and treatments
- Travel between patient homes using personal vehicle (no mileage reimbursement provided)
Requirements:
- Valid Tennessee state license or compact license
- Minimum 1 year of home health experience
- BLS certification
- OASIS training and experience
- Valid driver's license and reliable personal vehicle
