Job Description

Job Title: Primary Care Nurse

Job Location: Remote (Primary need in California, Texas, and Massachusetts)

Length of Assignment: 13 weeks

Start Date: Early March

Work Schedule: Initially 12 hours/week, increasing to 20 hours/week in, with potential for full-time

Roles and Responsibilities:

- Conduct Medicare Annual Wellness Visits (AWVs) in compliance with HEDIS standards

- Provide preventative wellness services and fill patient gaps in screening

- Complete 90%+ of work via asynchronous file review process

- Review and prioritize work queues for patient enrollments

- Participate in future meetings to discuss process improvements and trends

- Collaborate with oversight physicians and healthcare team members

Requirements:

- Active RN license in California, Texas, and/or Massachusetts (multiple licenses preferred)

- Minimum 2 years of experience with Medicare Annual Wellness Visits and HEDIS compliance

- Experience in population health and preventive wellness services

- Proficiency in asynchronous telehealth and file review processes

- Familiarity with Athena EMR (preferred)

- Ability to work independently in a remote setting
