Barton Healthcare Staffing is seeking a qualified Social Worker for a travel assignment at a hospital in California. The ideal candidate will be responsible for providing comprehensive social work services to patients and their families.
Responsibilities:
• Assess patients' psychosocial needs and develop appropriate care plans
• Provide counseling and support to patients and families
• Collaborate with interdisciplinary healthcare teams to ensure holistic patient care
• Assist patients with discharge planning and coordination of community resources
• Maintain accurate and timely documentation of patient interactions and interventions
• Advocate for patients' rights and needs within the healthcare system
• Participate in case conferences and team meetings
Requirements:
• Master's degree in Social Work
• Current California state licensure as a Social Worker
• Minimum of 2 years experience in a hospital or healthcare setting
