Job Description

Job Title: Social Worker

Job Location: Massachusetts

Type of Facility: Outpatient Multi-services Behavioral Health Facility

Length of Assignment: 26+ weeks

Start Date: Immediately

Work Schedule: Monday - Friday, 9AM - 5PM


Roles and Responsibilities:

- Conduct in-person behavioral health visits for adult outpatient population (18+)

- Participate in morning huddles with care team to discuss patient charts

- Manage and follow up on 12-15 patients per day

- Provide co-occurring substance use disorder (SUD) treatments for outpatient adult behavioral cases

- Work independently while collaborating with a team of 4-5 therapists, 1 MD, 1 NP, and 3+ RNs

- Utilize Ahima EMR system

- Potential to float


Requirements:

- Master's level therapist/substance abuse counselor (LCSW, LICSW, LMHC, or LADC I)

- Valid Massachusetts state license

- ACLS and BLS certifications

- 2+ years of experience in outpatient behavioral health for adults

- Comfort working as charge and in independent practice

- Experience with coordinated care for co-occurring mental health and substance abuse disorders
