Job Description

Job Title: Social Worker

Job Location: Nebraska

Type of Facility: Acute Care Hospital

Length of Assignment: 13 weeks

Start Date: Late May

Work Schedule: Monday-Friday, 8am-5pm


Roles and Responsibilities:

- Provide social work services in an inpatient behavioral health setting

- Conduct individual and group therapy sessions

- Collaborate with a multidisciplinary team including RNs and activities coordinators

- Develop and implement treatment plans for patients aged 55 and older

- Address major diagnoses including neurocognitive disorders, dementia with agitation, depression, suicidal ideation, bipolar disorder, and schizophrenia

- Assist with discharge planning and care coordination

- Utilize Epic EMR system for documentation

- Adhere to all hospital policies and procedures

- Maintain patient confidentiality and uphold ethical standards


Requirements:

- Valid Nebraska LCSW license

- Master's degree in Social Work

- Experience working with geriatric populations and behavioral health issues

- Strong communication and interpersonal skills

- Ability to work independently and as part of a team
